Below is a copy of the Clubs Proxy Voting Form to enable you to allow members to vote on important Club matters when unable to attend the vote in person.

The completed form must be given to the secretary before the commencement of the general meeting for which the proxy is appointed.

A form appointing a proxy sent by post or electronically must be received by the Association no later than 24 hours before the commencement of the meeting for it to be effected.
